2. Application Letter (Typed)
3. Photocopy (Coloured) of Passport Data page or any identification to confirm nationality.
4. Police Report (For lost passport).
5. Two recent passport-sized coloured photographs on a white background.

7. Payment of Rm 300 to Revenue Account, CIMB Acct. No. 8000317381 (NO CASH PAYMENT)
8. Payment of  Rm 130 Administrative Charge fee to the Nigerian High Commission’s Administrative Charge Account CIMB Acct. No. 8011445599 (NO CASH PAYMENT)
9. Kindly come along with proof of payments/Payment Slips
